I love doing that kind of thing with photoshop. I have a friend who loves P.O.D so I took a picture of his face and put him on the cover with them. If you didnt know how P.O.D was you wouldnt be able to tell something was up.

Seriously though, you take a high res picture of someones face and scale it down so its the same size as the person your removing the head from. Then you remove any area of the underlying image that you can see with the clone tool. The clone tool comes in handy because it can be used to make erased areas look like a part of the background. For example in my image the clouds and cliffs to the right of my friend actually werent in the image. They were cloned. From there its all about the little things like adjusting the lighting so it matches the lighting in the image behind it. In the case of the POD pick a lot more had to be redone because Sonnys dreads were all over the place so i pretty had to reconstruct his shirt...and right arm.
