However, it hasn't yet.
You have to recognise the current most commonly accepted definition.

it’s all about the context the word is used in. A word is a word, how it is used determines the meaning.

i guess i think anybody who lets being called a fag by some angry teen bother then is partly to blame for their own trauma.
i mean the guy saying it to you is probably worthless anyway. i've been called meaner shit then that in my lifetime by people i knew to be garbage and i turned out ok.
plus, as chris said, context is important. i would say that unless a guy was saying "I hate gay people! I want to kill all fags!" that he is probably not spreading hate if he happens to say something like "Let's see how much Parcells wins this year. I'll make him pay when we play them. The homo."
I don't think Jeremy Shockey was spreading hate. he was just having fun. Context is important.
